Cleveland Police identified the 12-year-old boy found wandering alone early Monday morning.
He was reported missing from his Garfield Heights home by his foster family.
Officers saw him walking on W. 130th Street near the I-71 on-ramp.
He was walking in the rain around 12:30 a.m. wearing a t-shirt and shorts, but no shoes.
Officers says he was unresponsive and unable to answer any questions.
He was taken to Fairview Hospital and then transferred to Cleveland Clinic's main campus.
The incident is still under investigation and no arrests have been made.
